Description
Canada, like other nations, faces major health care challenges with regard to its aging population and an increasing burden of chronic disease. The hospital-centric care delivery model of the last century is no longer appropriate to address today's urgent health needs. The symptoms that flag the need for transformation are evident: high rates of emergency department visits for conditions that are better managed in primary healthcare; poor access to timely first line health services; social, cultural, and environmental conditions that make unhealthy lifestyle choices the easier choices; and higher rates of injury and mortality for vulnerable subgroups of the population.
Improved health and chronic disease prevention management, and reduced inequities in access to care, are within our reach if new approaches to patient and family-centered CBPHC are developed through interdisciplinary, inter-professional, and community-based partnerships.
The inter-disciplinary research teams supported through these CIHR Innovation Team Grants will seek to do just that. Their research will aim to improve the delivery of CBPHC across Canada, with the ultimate goal of improving the health and well-being of Canadians and other citizens. This CBPHC research will focus on nation-wide challenges such as chronic disease, access issues for vulnerable populations, increasing health care costs, and underperforming health care systems.
The CBPHC Innovation Teams are the first component of CIHR's Signature Initiative in CBPHC. The initiative's goals are to: improve access to appropriate community-based primary healthcare; enhance the patient experience; and contribute to better health outcomes. The second component of this initiative will support CBPHC Health Professional Scientists.
