Checklist for Changes Pertaining to the Location of Tenure, including Proposed Relocations to Institutions Outside of Canada, and/or the Course of Study and/or the Supervisor for Training Award Recipients

Instructions:

  1. Assemble the request package in the order listed below.
  2. Submit the request package to "CIHR" at least three (3) months prior to the effective date of the proposed change.

From the Nominated Principal Applicant:

  • A signed letter, including:

For changes pertaining to the research project only:

  • a description of the new research project, including a project title (maximum one page).

For a change pertaining to the location of tenure only:

  • to the effective date of the proposed change;
  • the location of the proposed new research environment (maximum one page);
  • a justification for the proposed relocation (e.g., the research environment is not currently available at the current location or in Canada). If the research environment is not currently available in Canada, include a description of the unique aspects of the proposed research environment (maximum one page); and
  • a completed and signed (original or copied) "Request to Change Location of Tenure for Training Award" form.

From the Proposed New Primary Supervisor, if applicable:

  • A signed letter of acceptance (maximum one page);
  • A Common CV (validated for CIHR), including their publication list (maximum of two pages).

From the Current Primary Supervisor:

  • A signed letter indicating their awareness of the proposed relocation, if applicable (maximum one page).
