Instructions to complete the ICS program application form when applying for an IMHA Community Outreach Grant

Please see the instructions below for completing the application form:

• Section 1a: Enter your CIHR PIN. If you do not have one, visit the new user registration page to register for a PIN and password.
• Section 1c: Affiliation - insert the name of your organization.
• Section 1d: Institution Paid – insert the name of your organization.
• Section 2b: Apply as an "Organization."
• Section 2e: Please indicate the total amount requested from CIHR plus the amount requested from other organizations if applicable.
• Section 2h: Not applicable.
• Section 2i: Please explain why the funding is needed for each item and how it will be used. A detailed budget that includes a justification for the use of funds, as well as a complete breakdown into components outlining the allocation of funds is required in the proposal.
• Section 3 (a-f): This section should not exceed 3 pages double spaced.
• Section 3d: The proposal must include a section on expected outcomes. This section should focus on identifying the long term impacts as well as evidence that the project will be completed as described.

You must send the following two formats of the ICS form to the Institute:

• Please email the ICS form and all the attachments to the Institute, IMHA@cihr-irsc.gc.ca.
• An original signed ICS form and all the attachments must also be sent by courier to the address listed below:
